Here you will find our online forms. Please fill out your patient history, Neurolens and electronic records authorization forms and send it to us at suarezopticalinfo@gmail.com or bring it in filled out the day of your appointment to ensure a speedy check-in process. 

 Once your prescription has been finalized and you are ready to order, simply give us your email address to register you online. Once you have been registered, check your email for your log in information and temporary password.
